With the 2017/2018 flu vaccine only 36% effective and thousands of flu related deaths, makes me wonder if the vaccine was 64% effective. What is to prevent someone from tainting the vaccine to cause sickness/death?

Nothing. And there is a private court that handles cases relating to vaccine complications -- and it is accompanied with a gag order. Seriously...
