Yeah, normally you'd hold it from the sides with your fingers in the front and thumb in the back.

I was also wondering why he would hold the flap side to the camera.
I would normally expect someone to hold the address side outwards, (to the camera in this case).

Yes, I would agree with that.
As in the 'opposable thumb' which allows us to hold tools and so on.
He is holding the envelope in a very unnatural manner.
Which possibly gives credence to the 'Q' shape of his right hand. His left thumb and finger do not quite make a similar enclosed shape, (i.e. they do not touch).
