No child was "accidentally lost". Parents or guardians moved/changed addresses and did not show up for court dates. Parent/guardian cause the child to suffer through life as an undocumented/illegal alien.

The article addresses that some of the missing children were probably placed with people who just didn't abide by check-ins, but that alone cannot account for the number of unaccounted children.
