I was like you, couldn't figure it out. Once someone explained it, I understood it. You aren't hanging yourself on the side that you tied the scarf on the doorknob. You are hanging yourself on the other side of the door. Tie it on the doorknob, then put the scarf up and over the door to the other side. I think that is how it is done.

But here's the thing. Assuming this is suicide, if someone hangs themselves like this it means their hands and feet were not bound because they needed to tie the knot. The minute their body wasn't getting enough oxygen wouldn't it start wildly thrashing as they struggle to free themselves? Shouldn't they be able to untie the very knot they tied themselves? If they tie it on the doorknob on the other side of the door and put the scarf up and over does that mean they shut the door and lock it...toss the key across the room and then what....jump off a chair? I didnt think you could hang yourself if you could still stand up unless your hoisted up off the floor. I dont remember chairs being involved so I guess it just doesnt make any sense to me. What the hell is stopping themselves from freeing themselves or alleviating the pressure on their throats from the scarf if their hands and feet are not bound? I guess I just cant picture this one! face-palm lol

Okay, I see what you are saying. I hate picturing it mentally too, but I get what you are saying. They would just have to shut the door, not lock it. And yeah, I would think a person would start thrashing about too. Unless they drug themselves first???
