The constitution makes clear that in the absence of the President, the Vice President Acts as President (and may become the President if there is no President). Assuming that the Vice President takes his oath of office as expected, the new Vice President would become President.

Not that is likely to happen but if needed all that would have to happen is DJT step down before that time, allowing Pence to become POTUS.
Pence could declare DJT as VP and then step down.
It is as likely as any of the other potential scenarios thrown out there (not very).
