Help an old lady out and get some magic
From an RP perspective I could NEVER give up the promise of immortality for an unknown reward, so without knowing the result I could not choose "no magic". From a meta perspective "no magic" provides EXACTLY what I want from the other choices in the CYOA in a way that would require at least 4 categories otherwise (talismancy for immortality, Ero+Familiar as a waifu option, Wish for shapechange).
Anyway, picking the better option (No Magic), so here are the choices:

Spellslinging is setup for general utility with a couple of emergency options in case something from across the veil were to bother us. Alchemy: Preparations just gives the largest pool of potential options, potion of transfigure is the one I probably want the most. The fact that it isn't permanent is one of the reasons why I had to go this route for the "ideal human body" instead. Rituals have a plan B and C in case basic ageless immortality fails, as well as teleport:other worlds because being stuck on one planet forever kind of sucks. Talismancy of course provides an ideal pocket dimension home with as much luxury as possible, hidden enough to be safe.

Summary (TLDR Version)
I reside in a comfy and well-defended magical city with unlimited major wishes. I work towards uplifting and spreading humanity throughout the multiverse.
Summary (Full)
This build is centered around wishes and creation. The focus talisman lets me turn my three one-time wishes into three annual lesser wishes. Those three lesser wishes can be combined into a larger wish that is 'almost' as powerful as a major wish. It will take many years of wishing, but my ultimate goal is to live a comfy and safe life in my pocket plane while guiding the world outside of my plane in the direction that I want it to go with my annual major wishes.
I took the option to turn my demesne into a pocket plane. My pocket plane lacks utilities, luxuries, and guardians, but all of that can be fixed with wishes. I will use a wish to create a small magical city inside of my pocket plane. The buildings and structures of that city will be non-euclidean thanks to the "bigger on the inside" demesne option. A small building may open up into a mall or become a hidden street filled with stores and attractions. With another wish I could fill my city up with inhabitants. These inhabitants will be philosophical zombies. They will have no conscious experience of their own, but will act as if they did. They will maintain the atmosphere of the city. They will be unaware of my presence and will treat me like a normal person. Since I can make it rain in my pocket plane and I can create any edible plant within minutes, generating food and water should be trivial.
I may occasionally invite living guests to visit the city and see the wonders, but I would not do this often, because I do not want to be sent to the supernatural side. I'd rather live comfortably in the mundane without having to worry about supernatural threats.
To achieve immortality I'll use wishes in place of a phylactery. "I wish that resurrection would be cast on me if I die" "I wish that I would be teleported to a safe location in my pocket plane if I die". Those two wishes should grant me self-resurrection. "I wish that my body would revert back to the body that I had when I was 16 years of age without altering my memories at the end of each year". That wish should extend my natural lifespan indefinitely.
My "senses" will let me know when a hostile invader has entered my demesne. I will have stored spellcharged items, so that I can summon an army of creatures to attack. Fatigue will be no issue since my mage tower makes spellslinger abilities draw power from the land rather than me.
My focus talisman lets me use omnimancy rituals without requiring components, and my mage tower dramatically reduces the casting time of those rituals. With the "create material" and "fabricate" rituals I can create modern weapons and spellcharged items for the police of my city to use. The inhabitants of my city will be non-violent unless attacked, but if a hostile supernatural invader enters my pocket plane the inhabitants of the plane will band together to defend the city. My personal home will be hidden from the inhabitants of the city by illusion magic. It will be protected by special guardians that are completely loyal to me. These guardians will either be a result of wish magic or the "create life" spell.
If the invader proves to be too much for my minions to handle my final trump card will be stored major wishes. Within a few decades I'll have saved up plenty of them, and will use as many as it takes to stop the invader.
Catalyze grants me magic resistance, invisibility, immunity to bullets, and immunity to mind influencing spells. Spellcharge grants me the ability to fly, blink, and phase through matter. Potions and rituals can rapidly heal any damage. Wish contingencies can cover other weaknesses like imprisonment and petrification.
If you took omnimancy and familiar magic you could use your familiar points to buy "Empowerment: Omnimancy" twice. That would give you 30 omnimancy points in total, which is enough for the omnipotence ritual. For your third magic category you could take wish magic and choose heart's desire. Rather than wish for omnipotence directly, you could say "I wish that I would succeed in the omnipotence ritual". That may be a possible loop hole.
I only took three categories - Omnimancy, Talismancy, and Wishmagic. Demesne is not a separate category of magic and the points I put into Familiarship come from the Omnimancy ritual "Summon Familiar".
Also, it is the Omnimancy ritual "Spellcharge Item" that grants me access to Spellslinger spells, if that's what you're talking about.

It specifies that it can't be a means to an end, it has to be the end. I'm thinking that kind of thing is still banned. Why people even bother making CYOAs where you can't choose to have fun is beyond me.
Can a Lesser wish extend the duration of an alchemist potion, say at least a year if not indefinitely? That seems to me to be a pretty constrained wish, only about the strength of what a normal school of magic could do condensed into a single use for a year. Mostly what I would want to do is make Transfigure permanent. I'd imagine Hearts Desire could probably do something like make all potions I use permanent.
I would assume that a major wish can permanently change your form. If so, I see no reason why a lesser wish could not change your form for a limited duration like a year. That's just my opinion though.
I said Lesser wish, the type you get from Gwenevere or via Eromancy or Implement+Wishmagic. 3 Wishes could pretty definitively change your form permanently on its own, might even be able to make you immortal.
I figure with a combination of a potion already granting whatever it is you are wishing for rather than having to wish the entire effect into being that a Lesser wish should be strong enough to "fix" that effect in place, ideally permanently until you use something else to replace or dispel it.
In the absence of the creator of this CYOA paying us a visit and telling us otherwise, I'd say you're probably right. A major wish has enough power to alter the minds of millions people, which would be necessary for "world peace", which was the example they gave for a major wish.
A lesser wish has about one third of the power of a major wish, which would still be a huge amount. When I think about it like that I don't see a reason why a lesser wish could not change your form permanently.

My build is centered around Summon Familiar ritual which grants me access to every category of magic. My focus will store all my rituals except Panacea, which will be needed if my Phylactery/Focus/Implement will ever be destroyed, and empower any magic that I gain with my familiar. I hope that my offspring and lovers will help me survive in case of destruction of Phylactery.
My first familiar will be used to gain Demesne with Mage Tower, after that another familiar will be summoned with 10 familiar points which will be used to gain Imbue Phylactery ritual and another Demesne with Mage Tower. After creating my Phylactery I will summon familiar with Wishmagic Empowerment and use my wishes to gain a lot of alchemical mystic materials. With the help from another familiar I'm going use them to Infuse myself, craft various Creations and Preparations. After all that I'm probably going to worldwalk either via Teleport ritual or Wishmagic and go on all sorts of adventures.

First, I'll choose to stay firmly on the Mundane side of the veil. Ain't gonna deal with supernatural shit. (Low Profile Is Best Profile.) The first type of magic I'll take is Eromancy. Supercharging my magic and all I have to do is strip? Yes fucking please. The Phoenix power, let's see, ten thousand orgasms had/caused will get me ten years, that's a thousand orgasms a year, 84 per month, 20 a week, or 3 a day, just for effective immortality alone, and anything on top of that juices up my other powers PLUS I get a Lesser Wish for a few thousand orgasms, why the fuck would I not?
The next category I'll take is Spellslinging. The first spell is Flight because Always Take Flight. With my Ero supercharge I can fly up to 5x running speed when nude, as I'm taking "more powerful" to mean. Locate will go well with flight as well. Invigorate/Exhaust, Charm, and Induce Emotion because if you're not taking these with Eroslinging you're a chump, it'll make getting a pile of orgasms so much easier. Also Clean"' is a must for any Eroslinging, for obvious reasons, and touch illusion Figments might be fun as well. Hell, Animate Objects would be fun too, animate a bunch of sex toys and those orgasms will rack up in no time. Transform with the nekkid boost should make it go from minor physical changes to major. Invisible, because Low Profile. Unlock, Phase, Shield, Knit Flesh, and Purify Food just in case I need to get out of a shitty situation. Telekinesis and Mage Hand''' just because I have the points left.
My third class of magic will be Alchemy. I'll learn Hihi'rokane Preparations, that lets me make Healing and Transfigure as better versions of what I already took in Spellslinging but I'll keep the spells because I can do those without prep. Sometimes redundancy is a good thing. Friendship and Tongues should help me get those 'gasms to power my mojo. Then Beastspeaker, Universal Solvent/Solution, and of course every Preparation type gets Lead Into Gold. Gotta love cash money. I'll take Hihi'irokane Creations too, it'll buff both Eromancy and Spellslinging. I'll learn Darksteel Preparations next, Water Breathing and Climate Proof will make traveling naked much easier. Cleanliness is a worse version of the spell because it wears off. Magic Resistance and Invictus mind control resist is pretty goddamn handy. Armor is redundant to my shield spell, but it has the advantage that I can protect someone else who is not with me. Orichalcum and Darksteel Creations because a no-maintenance vehicle sounds brilliant to me. Tune it up, imbue it, it just fucking works forever. Orichalcum, Starmetal, Hihi'irokane and Moonsilver Infusions, THIS IS WHAT PEAK PERFORMANCE LOOKS LIKE MOTHERFUCKERS. Top tier in body, mind, senses, and magic. I'm ready to roll now.

By the way, did you read well? Phoenix prolongs your lifespan by 10 years only if you die of old age. That means you'll be a permanently old man, and only so long as you can keep cumming regularly at that age (good luck with that). Also, just to clarify: you pay for Lesser Wishes separately (with additional orgasms) so you'll have to cum even more.

There's a good chance sex will be the last thing on their mind after seeing you, and turning that to lust will totally exhaust you. Speaking of which, Invigorate/Exhaust can't be used on self and only has effect for a few minutes (thus making it useful mostly for emergencies).

Well yes, I'll be an old man, but I'll be an old man at the peak of human fitness, attractiveness, and wits thanks to all the infusions. They don't need to be MY orgasms, if I need some magic viagra I have a half-dozen ways to accomplish that, and with transformations I can look like whatever floats anybody's boat. As for the magic/mind control resistance, A: it came with the Darksteel Preparations, B: I'd be an idiot to assume the only one on the mundane side who knows magic, and C: I'd also be an idiot to assume there are no assholes among these magic users.
As for my plan, you overlook a few fundamental points. One, I may become an old man but thanks to Lead Into Gold I will become a very WEALTHY old man. Two, I will be able to bless people with peak physical and mental fitness. I could be a mummy and gold diggers would still be all over my jock looking for some easy cash and a magic liposuction.
It's pretty much lore for Troy's Veil to be separating the mundane and magical almost completely and for the "straddlers" to be dying out โ pretty much every time you're described as the first one in centuries (if not millennia) to even exist.
And really, there's at least several ways to get eternal youth plus several kinds of magic, so this grand plan of yours to eternally die of old age doesn't look exciting in the least.
I refer you to A: it was a side-bonus of something I was getting anyway, and must also point out that I will BE THERE in millennia when the next one shows up. Also, if you don't think basically being Papa fucking Emeritus disguised as Hugh Hefner isn't exciting, I must simply disagree.
