The therapists are not properly prepared to treat psychotic patients.
Freud famously said they can't be treated.
Talking therapy doesn't work until they are quite a ways into remission, which does happen.
Thought doctors are taught that the psychotic patient will never recover.
If one does recover, they change the diagnosis

If it changes they just adjust the measuring stick.
An ordinary doctor or shrink can absolutely not treat such patients. They need a specialist.

It often just casually means 'delusional" or "illogical" or "emotional"
for most people have never been to a hospital where the patients are off meds.
Have never seen it up close.
Even the docs and nurses who do see it, have little idea what it really is
And so yes, it scares people.
