Pharmacy is mostly governed on a state-to-state basis. Here in Texas, as far as I know, there is nothing that legally requires a prescriber to indicate why they're prescribing a certain medicine to a patient. In my experience, when we're talking about drugs that are not controlled substances, the insurance companies (including Medicaid/Medicare) are the ones imposing the restrictions and red tape.
