FDA Adverse Event Reporting System is not a valid source for drug safety data

https://www.fda.gov/drugs/questions-and-answers-fdas-adverse-event-reporting-system-faers/fda-adverse-event-reporting-system-faers-public-dashboard

The FAERS Public Dashboard is a highly interactive web-based tool that will allow for the querying of FAERS data in a user friendly fashion. The intention of this tool is to expand access of FAERS data to the general public to search for information related to human adverse events reported to the FDA by the pharmaceutical industry, healthcare providers and consumers.

While the FAERS dashboard offers stakeholders many more ways of searching for and organizing data on adverse events reported to the FDA for many drug and biologic products, there remain limitations to the data. For example, while FAERS contains reports on a particular drug or biologic, this does not mean that the drug or biologic caused the adverse event. Importantly, the FAERS data by themselves are not an indicator of the safety profile of the drug or biologic. Some additional limitations to note include:

Duplicate and incomplete reports are in the system: There are many instances of duplicative reports and some reports do not contain all the necessary information.

Existence of a report does not establish causation: For any given report, there is no certainty that a suspected drug caused the reaction. While consumers and healthcare professionals are encouraged to report adverse events, the reaction may have been related to the underlying disease being treated, or caused by some other drug being taken concurrently, or occurred for other reasons. The information in these reports reflects only the reporter's observations and opinions.

Information in reports has not been verified: Submission of a report does not mean that the information included in it has been medically confirmed nor it is an admission from the reporter that the drug caused or contributed the event.

Rates of occurrence cannot be established with reports: The information in these reports cannot be used to estimate the incidence (occurrence rates) of the reactions reported.

Patients should talk to their doctor before stopping or changing how they take their medications.

Improving data access and transparency are core concepts that drove the development of this FAERS Dashboard. The FDA anticipates that this increased transparency will help to spur the submission of more detailed and complete reports from consumers, health care providers and other members of the public. Complete and detailed reports are immensely helpful to the agency when identifying safety signals and choosing particular products for further scrutiny.

Also, apparently this data includes foreign input. A large amount of reported events are not actual medication reactions, but commentary on things like effectiveness.

Lawyers call to grant Julian Assange's 'long overdue freedom'

https://www.tvnz.co.nz/one-news/world/lawyers-call-grant-julian-assanges-long-overdue-freedom (Sun, Aug 16)

Julian Assange's freedom is long overdue, according to a group of lawyers and legal academics who have called on the UK Government to end extradition proceedings against the Wikileaks founder and release him from prison.

Assange is fighting to avoid being sent to the US to face 17 charges under the Espionage Act and conspiracy to commit computer intrusion after the publication of hundreds of thousands of classified documents in 2010 and 2011.

In a letter to Prime Minister Boris Johnson, Justice Secretary Robert Buckland, Foreign Secretary Dominic Raab and Home Secretary Priti Patel, 169 individuals and legal organisations called for the government to intervene.

If extradited, campaigners said the 49-year-old will face a "show trial" in the US.

They added he has been subject to surveillance which violates his right to a fair trial.

"We call on you to act in accordance with national and international law, human rights and the rule of law by bringing an end to the ongoing extradition proceedings and granting Mr Assange his long overdue freedom," the letter signed Lawyers for Assange, reads.

The group said the political nature of his alleged offences prohibits his extradition under the US-UK extradition treaty and that UK judges in his case have been subject to conflicts of interest.

They added Assange is at risk of torture or cruel, degrading and inhumane treatment in the US, citing the potential 175-year sentence.

In June, more than 200 doctors from 33 countries signed a letter published in The Lancet, accusing UK and American officials of "intensifying Julian Assange's psychological torture".

Assange has been held in Belmarsh Prison in south east London, awaiting proceedings after serving a 50-week sentence for breaching his bail.

Assange's extradition hearing, originally set for May, was postponed as a result of the coronavirus lockdown measures and was rescheduled for September 7.

The open letter comes after the US made a fresh extradition request with a third version of the charges against him on Friday.

On Dr. Scott Atlas, the anti-Fauci

In private meetings at the White House, Atlas has irritated other aides by arguing against expanded Covid-19 testing. He opposed a proposal championed by Dr. Deborah Birx, the response coordinator for the White House coronavirus task force, to scale up home testing through methods such as saliva tests. And recently, in a task force meeting, he told Dr. Anthony Fauci, the nation’s top infectious disease expert, that science does not definitively support government mandates on wearing masks. (The Centers for Disease Control and Prevention recommends that Americans wear masks when they interact with those outside the home and in instances when social distancing is not possible.)

A few weeks ago, Atlas officially joined the administration as an adviser, and in a short time he has become a frequent presence in the Oval Office and around the White House complex.

He spoke publicly at a White House event about school reopening and appeared at one of the president’s evening briefings, the only medical professional to do so in recent weeks. He has quickly established himself as a voice pushing for the resumption of daily activity, including college football and schools — which Trump advisers and aides see as a key marker of normalcy that will help Trump’s reelection campaign.

Far more significantly, he’s part of a tiny group of advisers who meet every morning to chart the daily response to Covid-19, a group that includes long-standing aides like Kushner and Stephen Miller. He has gained influence so quickly that he even helps to prepare the president in the Oval Office for the newly revived evening briefings and makes suggestions for Trump’s opening remarks, according to interviews with six senior administration officials and Republicans close to the White House.

Atlas frequently questions or spars with other administration officials about data on the spread of the virus, or the efficacy of the government’s requiring people to wear masks, or the merits of broadening testing among the wider population — all of which other health professionals consider key planks in combating the virus, a sort of Pandemic 101.

He has become the president’s go-to Covid-19 doctor, the anti-Fauci, even if he does not have a background in infectious diseases or epidemiology. Instead, his specialty lies in radiology and neuroradiology, subjects he taught for many years as a professor and chief of neuroradiology at the Stanford University Medical Center.

“There is nothing inherently bad with having a management leader of a task force who is not a subject-matter expert, like an epidemiologist,” said Juliette Kayyem, a former top Obama Department of Homeland Security official who was heavily involved in the response to the H1N1 pandemic.
