The novel coronavirus, COVID-19, is a worldwide pandemic.. COVID-19 is extremely contagious. COVID 19 can cause serious illness and is especially unsafe for people at an increased risk of severe complications. As a result, federal and state governments and health authorities have recommended a number of mitigation strategies including the use of face coverings, social distancing, avoiding gatherings, avoiding use of shared equipment, and healthy hygiene measures including proper handwashing and using hand sanitizer.

 

 

The University of Virginia Intramural-Recreational Sports Department (IMREC) has put in place mitigation measures attempting to limit the spread of COVID-19, but IMREC cannot guarantee that you will not become exposed to or infected with COVID-19. The use of IMREC facilities and participation in IMREC programs and services will increase your risk of contracting COVID-19. Knowing this, you agree as follows:

 

 

I acknowledge the contagious nature of COVID-19 and voluntarily assume the risks that I may be exposed to or infected by COVID-19 through my use of IMREC facilities and my participation in IMREC services and programs.

 

I understand the risk of becoming exposed to or infected by COVID-19 through my use of IMREC facilities and my participation in IMREC services and programs may result from the actions, omissions, or negligence of myself and others.

 

I agree that I will follow recommended safety and hygiene guidelines, including all signage and directions, established by IMREC and the University of Virginia, to limit my exposure and the exposure of others to COVID-19.

 

I attest to the best of my knowledge that I have not had exposure to any individual who has had an expected or confirmed case of COVID-19 in the past 14 days. In addition, I agree that I will not use IMREC facilities or participate in IMREC programs or services until I am medically cleared, if I (i) am experiencing symptoms of COVID-19, including, without limitation, fever, cough, or shortness of breath, or (ii) have a suspected/confirmed case of COVID-19.

 

By signing this Acknowledgement and Assumption of Risks, I knowingly and voluntarily assume all risks and accept sole responsibility for any injury to myself, illness, disability, liability, or expense, of any kind, that I may experience or incur in connection with or as a result of my use of IMREC facilities or my participation in IMREC services and programs.
