I would say that because bacterial pneumonia can follow after/with viral pneumonia that those 60,000 COVID +[bacterial] pneumonia deaths are attributable to the virus. COVID initiates a breach in local lung tissues setting up likelihood for bacterial infection.
It is the 100,000 other deaths that are suspect.
A weird thing about COVID is it predisposes to blood clots where you do not want them. It would be hard to exclude those from the 100,000.

It’s not the doctors. They aren’t influenced in any way how they fill out death certificates. Somewhere down the chain in each states system of records keeping I would guess.
You are correct that sometimes it is an “educated” guess with one initial author. Not a foolproof system by any stretch.
