One is responsible only for the acts he deliberately commits, less so for innocent mistakes which result in harm, and not responsible for errors of omission.

To carry the idea that you are responsible for things you could have stopped, but didn't, extends that field so far as to encompass everything in the world that goes wrong.

How can you determine whether you would have been personally able to stop something from happening?

"If only I would have done something" only serves to fill you with guilt, and never can make things different except in a parallel universe.
