In Minnesota, people have received 6 to 8 Vote at home ballot request forms, with info already typed in with the exception of last 4 digits of SS, Signature and of course empty line to fill out where you want your ballot sent to.

Doubt they are going to cross check any of these. Burn them and vote in person at the polls!
