Look at all the notches in the slats on the left wing. Big chunks missing.
The engine throws the blade off the fan, which chewed the cowling, sending chunks everywhere, as luck would have it, not directly at the fuselage.
Big chunk goes over the wing, cuts/breaks the plastic window, pressurization blows the outer window, takes the inner with it, along with an unbuckled passenger.
