Yes, all those who have Jain as their family name basically belong to Jainism religion. It is a different matter how much they practice or follow Jainism.

Jain is originally not a family name. It has been adopted as a surname or family name by many of the followers of Jainism. From time to time there have been campaigns that all those following Jainism should write Jain as their family name and I know many Jains who have started using Jain as their last name instead of their earlier family name. (There are many others who follow Jainism but do not write Jain as their family name or surname).

So anyone who uses Jain as a family name definitely has a Jainism lineage. May be over a period of time someone in the family starts following some other religion and then for generations to come they continue using Jain as a surname while following a different religion. But such cases may not be many. I have never come across any such case.

Some Jains, while following Jainism, can be seen going to Hindu temples too. They have faith in Hindu deities. But if their surname is Jain they certainly come from Jainism heritage.
