Groups targeted for vaccination expanded from high-risk children only to including all children aged 6–23 months (starting as “encouraged when feasible” in 2002 and recommended since 2004) [8–10], children aged 6–59 months (since 2006) [11], and children aged 6 months to 18 years (since 2008)
