I think if they deployed something like that it would be to deliver a payload (bioweapon?) and wouldn't have been done while he was on camera and under the lights against his white hair where it could be spotted by secret service or anyone watching a camera feed locally. The fly that appeared on the broadcast was made obvious to everyone watching.
I think it's more likely it was digitally added as a cheap campaign stunt so Biden campaign could make it a smear of Pence and make a joke out of promoting the fly swatters, which they conveniently had ready to go in their online store within 45 mins of the end of the debates.
Would be curious to find whether it's reasonable to find a supplier that could source something that fast, or better yet, if the one they used has a record of Biden campaign ordering it in advance.
