Your vote is supposed to be secret. So the ballot should have no personal identifier.
And that's the problem with mail in voting. What you describe could happen.
You may be able to see what each state has certified on their election site. Usually it's the SOS in charge of that. They may certify several systems. As for each county, again, state may have that information.
