Hospitals require that upon discharge from an admission. Not required generally for leaving an office visit.

At hospitals I've worked at, in ER, it depends on pt. situation. On discharge after an admission & hospital stay it is mandatory.
I agree with you, Anon. Just stating usual hospital standards. Further, the post says he was leaving and Orthopedic office. Different standards there. And it shows him apparently standing but we don't know if he's on crutches, either.
