Looks to me like a PS edit.

I think Pence was taken out, and Pompeo and Trump were masked up and moved. Urn on the fireplace left was cloned from the one on the right, and the walls either side were brushed up. Pretty simple edit.

Easier edit that way because of the Flag on the right, which would have been harder to match up with Trump's left shoulder (right as we look at it).
