Standard for extremely wealthy individuals to have an attorney fixer that has discretion to pay nuisance people to go away, without having to get permission/approval from client up to a certain amount. The amount paid is billed to the client with the legal fees/billable hours, and the client's financial people pay the bill without the client ever even knowing about it. Someone like Trump doesn't pay his bills, he has people to handle all that stuff.
So, he didn't know about the settlement ($130K not important amount to him, it's like $5 to you and me), he didn't know about the "reimbursement" as in the legal bill being paid, and it wasn't out of campaign funds so it's NOT RELEVANT ANYWAYS.

You don't harvest organs and then ship them like that. Most organs have a short window of viability once removed from the body, just a few hours at most. That is why private transplant patients travel to the country where the organ is, to get the transplant.
