Look at the bone structure. The contour of the temple. The proportional relationship to the cheek bones, and the chin. The spacing of the eyes, and bridge of the nose. (None of which can be altered via surgery.)
Then you consider the soft tissue. The shape of his nose, the ears, and the chin. (he now has a "butt" chin, where he didn't before) While these things could potentially be done via surgery, nobody would pay to enlarge their ears, or attach their lobes.
