The transcriptionist, Kimberly Nolan, misspelled her own name in the signature and had to insert an "R" into it afterwards to correct?
I was a transcriptionist for more than 20 years. I find it odd that she didn't just reprint the form and redo her signature correctly before submitting the finished product to the client.
