You could just print it on ordinary paper fill it out and then xerox the two parts so they look separate, maybe slightly crooked, and hand employer xeroxed page. say you're keeping the original in a safe place.

I'm guessing the employer probably doesn't want to be the cause of a spike in cases, and get shut down and wants his ass protected. Hopefully they just shove a note saying vaxed in the personnel files and forget about it unless there is some "state health" investigation down the road.
