Formulated to reduce population but not so much that it’s blatantly obvious that population reduction is the intent. I’m sure the mortality rate is engineered to be substantially higher than with random side effects of other drugs and vaccines, and there’s probably solid genetic science behind the engineering such that they can pretty much predict the outcome for certain genetic makeups (e.g. people in the population with these genetic markers are much more likely to suffer from adverse effects than those with different genetic markers). While the engineering likely targets a much higher mortality rate than is typical with most other drugs’ side effects, it’s probably done so that there are still few or no red flags. With a complicit media and a high percentage of the populace with dulled senses, this is more than just possible, it’s actually quite plausible.
