All employees must receive the first of a two dose covid vaccination by June 15th with the second, final shot, no later than July 26st . If opting for the one-shot vaccine, that vaccine should be administered no later than June 15th . Individuals seeking an exemption from this requirement for medical or religious reasons should complete a request for an accommodation form and submit the form to the human resources department.
