Dreams come from mental activity, but since the mind is non-physical you can't kill it physically. You have to stop thinking, basically.
It doesn't have to be. That part is subjective. My father once cut his leg very badly with a chainsaw. He looked at it as a chance to learn how the doctors stiched up such a nasty wound, instead of panicking.
