My understanding is two ways.

 

  1. You can 'present' with symptoms, i.e. you are physicially sick. In a normal environment the doctor would have written 'flu' as the diagnosis, but in the Covid environment he writes 'Covid-19'. If there is a known ongoing epidemic of 'cases', they don't even need a test to confirm. They may or may not order a test just to collect data.

 

  1. You can be tested without symptoms, i.e. you were 'exposed' to someone, and have been asked to undergo testing. Under the old PCR threshold protocols, you would be highly likely to be 'positive'. You would then be considered an 'asymptomatic carrier'.
