all of those school / work compliance forms are being signed under duress, because they threaten you on the face of it with a deadly disease, with no proof, then require that you either show proof of compliance or sign an "acknowledgement" or lose your job or schooling.
because it is under duress, it cannot be notarized. because it cannot be notarized, it is only hearsay.
from there, anyone "requiring" a document signed under duress probably guilty of some kind of fraud.
