We’ll that’s federal so it’s different but every single state has a religious exemption. Some have more stringent requirements than others (e.g. clergy signature, supporting documentation, notarization). Here in NC you simply have to submit a note to your employer that you are claiming a religious exemption. No signatures, no supporting evidence, just your own statement.
