Imagine if Q wanted to do this type of operation in the 70s. He couldn't "hear you breathing", he wouldn't be able to see all.

Imagine if Q wanted to do this in the middle ages. You would have to have literal eyes and ears everywhere and would need to trust the chain of custody of information all the way from start to finish.
