There is a committee that draws up a short list of those eligible. AFAIK, only a higher franking general or perhaps only the POTUS can make the actual promotion. When I lived on Torrejon AB in Spain ('83/'84) President Reagan called during the Octoberfest celebration and promoted our Full Bird Colonel base commander to Brigadier General over the building wide intercom. Prolly the Deep State these days though.
