Doctors keep two sets of notes, one that patient usually gets as "discharge" or "visit summary", then they have the notes that go into your personal medical file. You can request the "private" personal file, usually at a cost of the copies. Learned that one a few years ago, visit notes said one thing whereas the notes in my med file told a slightly different more specific story.

No, they cannot hide your medical records from you, that is against the law. If someone else were interested in those files, such as a for a court case or some such, they would need a subpoena
