Matrix Medical Network “changes lives” by providing in-person health risk assessment services and data analytics that help Medicare Advantage plans and their members. These assessments are one-on-one, in-home health evaluations conducted by a licensed health care provider that identifies risk factors that should receive further medical evaluation. During the visit, Matrix’s employee Nurse Practitioners perform a non-invasive physical exam, ask questions about personal, social and family medical history, and review the member’s living environment and current medications. In addition, the NPs provide health information and make recommendations for further evaluation. Matrix began providing care management services in 2001, and pioneered the use of a national network of highly trained, employee NPs to conduct health assessments in the member’s home or nursing facility. In 2012, the company completed more than 224,000, 60-minute-plus assessments. Matrix is one of the largest providers of prospective health assessments in the country.
