Projections of global mortality and burden of disease from 2002 to 2030

Abstract

Background: Global and regional projections of mortality and burden of disease by cause for the years 2000, 2010, and 2030 were published by Murray and Lopez in 1996 as part of the Global Burden of Disease project. These projections, which are based on 1990 data, continue to be widely quoted, although they are substantially outdated; in particular, they substantially underestimated the spread of HIV/AIDS. To address the widespread demand for information on likely future trends in global health, and thereby to support international health policy and priority setting, we have prepared new projections of mortality and burden of disease to 2030 starting from World Health Organization estimates of mortality and burden of disease for 2002. This paper describes the methods, assumptions, input data, and results.

 

The three leading causes of burden of disease in 2030 are projected to include HIV/AIDS, unipolar depressive disorders, and ischaemic heart disease in the baseline and pessimistic scenarios.

 

Conclusions: These projections represent a set of three visions of the future for population health, based on certain explicit assumptions. Despite the wide uncertainty ranges around future projections, they enable us to appreciate better the implications for health and health policy of currently observed trends, and the likely impact of fairly certain future trends, such as the ageing of the population, the continued spread of HIV/AIDS in many regions, and the continuation of the epidemiological transition in developing countries. The results depend strongly on the assumption that future mortality trends in poor countries will have a relationship to economic and social development similar to those that have occurred in the higher-income countries.

Inspired by a parasitic worm that digs its sharp teeth into its host's intestines, Johns Hopkins researchers have designed tiny, star-shaped microdevices that can latch onto intestinal mucosa and release drugs into the body.

 

David Gracias, a professor in the Whiting School of Engineering, and gastroenterologist Florin M. Selaru, director of the Johns Hopkins Inflammatory Bowel Disease Center, led a team of researchers and biomedical engineers that designed and tested shape-changing microdevices that mimic the way the parasitic hookworm affixes itself to an organism's intestines.

 

The "theragrippers" are made of metal and a thin, shape-changing film, then coated in heat-sensitive paraffin wax. The devices, each roughly the size of a dust speck, can potentially carry any drug and release it gradually into the body.

 

The team published results of an animal study this week as the cover article in the journal Science Advances.

 

Gradual or extended release of a drug is a long-sought goal in medicine. Selaru explains that a problem with extended-release drugs is they often make their way entirely through the gastrointestinal tract before they've finished dispensing their medication.

 

"Normal constriction and relaxation of GI tract muscles make it impossible for extended-release drugs to stay in the intestine long enough for the patient to receive the full dose," says Selaru, who has collaborated with Gracias for more than 10 years. "We've been working to solve this problem by designing these small drug carriers that can autonomously latch onto the intestinal mucosa and keep the drug load inside the GI tract for a desired duration of time."

 

Thousands of theragrippers can be deployed in the GI tract. When the paraffin wax coating on the grippers reaches the temperature inside the body, the devices close autonomously and clamp onto the colonic wall. The closing action causes the tiny, six-pointed devices to dig into the mucosa and remain attached to the colon, where they are retained and release their medicine payloads gradually into the body. Eventually, the theragrippers lose their hold on the tissue and are cleared from the intestine via normal gastrointestinal muscular function.

 

Gracias notes advances in the field of biomedical engineering in recent years.

 

"We have seen the introduction of dynamic, microfabricated smart devices that can be controlled by electrical or chemical signals," he says. "But these grippers are so small that batteries, antennas and other components will not fit on them."

 

Theragrippers, says Gracias, don't rely on electricity, wireless signals or external controls. "Instead, they operate like small, compressed springs with a temperature-triggered coating on the devices that releases the stored energy autonomously at body temperature."

 

The Johns Hopkins researchers fabricated the devices with about 6,000 theragrippers per 3-inch silicon wafer. In their animal experiments, they loaded a pain-relieving drug onto the grippers. The researchers' studies found that the animals into which theragrippers were administered had higher concentrates of the pain reliever in their bloodstreams than did the control group. The drug stayed in the test subjects' systems for nearly 12 hours versus two hours in the control group.

 

The technology is available for licensing through Johns Hopkins Technology Ventures.

The Pentagon’s RESPONSE to the explosive DOD medical data is an even bigger story than the data

One thing is clear about the revelation of the 2021 military epidemiological data and the military’s response to it: There is undoubtedly a public health and national security crisis in the military, and the Pentagon’s reaction only seems to be concerned with exonerating the vaccine, not fixing its own alleged problem.

 

It’s now certain that the military’s health surveillance system — DMED — showed a massive increase in sickness and injury diagnoses in 2021 over previous years, particularly in the neurological, cardiovascular, oncological, and reproductive health categories. The military, in a very terse and cryptic statement to PolitiFact last week, admitted as much, but claimed without any further explanation that the data in the system accessed by several military doctors working with attorney Thomas Renz was only a “fraction” of the true numbers that existed. In the words of the Pentagon spokesman, it was a “glitch in the database.” Where those true numbers existed, why they weren’t in the system for five years, what exactly was in the system, and why the 2021 numbers were accurate according to the DOD account remain a mystery.

 

However, one by one, the military public health officials have been adding back random numbers to the 2016 through 2020 codes. I’m told by Renz and two of the whistleblowers that throughout the past week, they have queried the same data again, and in most of the ICD categories, they have found that the numbers from 2016 through 2020 were “increased” exponentially to look as though 2021 was not an abnormal year. This has been done without any transparency, any press release, any statement of narrative, and sloppily in a way that makes the already unbelievable narrative simply impossible to believe.

 

In addition to believing that every epidemiological report for five years was somehow completely tainted with false data — including during the first year of the pandemic itself — we would have to believe that the minute they discovered this from Renz, they suddenly discovered the exact numbers. A five-year mistake fixed overnight!

Denmark officials see no reason to give more COVID vaccines

 

COPENHAGEN, Denmark (AP) — Health authorities in Denmark said Friday that they were considering “winding down” the country’s coronavirus vaccination program in the spring and see no reason now to administer a booster dose to children or a fourth shot to anymore residents at risk of severe COVID-19.

 

The Danish Health Authority said in a statement outlining its reasoning that the third infection wave in the European nation was waning “due to the large population immunity.”

 

“The very high vaccine coverage in Denmark, especially with the third shot, means that we can cope with increasing infection without getting serious illness,” the government agency said in a statement.

 

Last month, the Danish government said it was offering a fourth vaccine dose to older adults and other vulnerable citizens because the pandemic situation had worsened amid the rapid spread of the omicron variant of the coronavirus.

 

But a Health Authority assessment concluded that three shots had provided good protection to nursing home residents and people over age 85, and the agency decided it was unnecessary to provide them with additional shots right now.

 

Bolette Søborg, a unit manager and chief physician with the authority, said the approaching end of winter, when time spent indoors makes it easier for the virus to spread, is another reason to hold off on fourth doses.

 

Only a handful of countries worldwide have started offering fourth shots or announced plans to do so.

 

Denmark expanded its vaccination program to children ages 5-11 in November, when the delta variant was dominant and there was a fear youngsters could infect older generations. Health authorities said Friday that they were “now starting to plan to round off the current vaccination program for all target groups, including the program for children aged 5-11.”

 

More than 80% of the population has received two shots while 61.3% have had a booster, according to official figures.

 

The agency said it was looking at “winding down the entire general vaccination program later in the spring.”

 

“We will of course follow the epidemic closely, and we are ready to change our decision if, contrary to expectations, there is a fourth spring wave or new worrying variants this spring,” the agency statement said.

 

Denmark scrapped most pandemic restrictions this month after officials said they no longer considered COVID-19 “a socially critical disease.” Officials said that while omicron is surging in the Scandinavian country, the variant was not placing a heavy burden on the health system.

 

This week, the country reported a slight drop in new weekly cases.

During quarter they did a $1.4 billion reinsurance deal with Wilton Re to offset larger policies in US due their increased mortality expectations in 2022. TransAmerica is owned by Aegon.

 

In the earnings transcript an exchange between CFO & Mr Huttner is illuminating: Basically as I read it they are doing an unfavorable deal to reduce volatility…obviously “volatility” is code for higher in their mind.

 

Question to CFO: “Given the vaccines & Omicron why do this?”
