My guess back then [at the rumored exfiltration around Christmas time (?)] was that he was flown out of a US base in England to Ramstein. Landstuhl & hospital right next door for any necessary medical procedures; plus used to high security (Benghazi injured were there). It's also a wooded area with lots of walking paths for convalescence.

Once there easy to hop to CONUS on a Ramstein flight when the time was right.

Just a guess though.
