Or use Form 433-F and mail it. This is either/or, chose the shortest form possible and do NOT volunteer excess information except the hardship aspect.
Make notes on the front- DISABLED/see notes and make a note/ add a note: "Alzheimers" and note the ages. IRS will not continue to harrass the elderly, the disabled.

What is the IRS Hardship Program?
Located within Part 5, Chapter 16, Section 1 [IRM 5.16.1] of the Internal Revenue Manual is the provision describing “Currently Not Collectable” or CNC status. Accounts can go into CNC status for a variety of reasons, but the stipulation listed for consideration of IRS hardship states, “collection of the liability would create a hardship for taxpayers by leaving them unable to meet necessary living expenses”.
What are the IRS Hardship Rules?
If your account is declared CNC under tax hardship, the IRS can no longer impose any collection procedures in an effort to settle your tax debt. Common collection methods include, but are not limited to: Lien/Levy/Garnishment (another word for Levy)
