During a race - probably involved some fence jumping. Pretty dangerous, steeplechase jockeys get badly injured often enough. Horse goes to jump, you lose your seat and off you go. Then a misplaced hoof on your person and it's goodbye. Sometimes the horse jumps badly and ends up on top of the rider. That's often fatal.

 

Just guessing, without more info. But foxhunting isn't allowed anymore, so… racing over the fences is the next best thing.
