In order for the TOTP to work correctly, the server needs to acknowledge two things:
1) the current time is the time when a data transfer begins, not the time when it completes
2) either the current TOTP or the previous is acceptable; this flexibility allows the expiry period to range between 30 and 60 seconds
Anything less runs into the problems that we are currently experiencing.
Jim suggested sending the password by email instead of just giving one out to all takers. This is obviously problematic. I doubt that this TOTP thingy is going to remain policy in the long run.
Try the extended version with multiple TOTP codes. Play the guessing game.

Of course not. Ad-blockers don't just block ads. They will block whatever has been blacklisted and whatever is considered suspicious. Dunno what might be considered "suspicious". Also, they may insert Javascript into the page and cause things to go hairy. It's always the first thing you should suspect when something doesn't work.
