In Canada this was done in 1981. References to the "Dominion of Canada" ended and substituted with just CANADA. Quebec (Nouvelle France) also formed an incorporated entity in 1981.
The "Charter of Rights" was their public announcement of this corporate reorganization and why Quebec has never signed it, even though they are subject to it under Color of Law.
New Brunswick is an interesting case as well. The Province of New Brunswick became the Government of New Brunswick, and there are still outstanding questions as to the status of Acadia, which was allowed for in the Royal Proclamation of 1763.
