I think trans people probably have some real alterations to their chemistry and possibly even their DNA.
Here's an example of a chemical thought to impact gender identity: https://diethylstilbestrol.co.uk/studies/des-and-gender-identity/
People on the autism spectrum seem to have gender dysphoria more than average: https://www.forbes.com/sites/zhanavrangalova/2017/11/15/growing-evidence-for-a-link-between-gender-dysphoria-and-autism-spectrum-disorders/
Now here's where it gets weird in my opinion. You know how a woman will have genetic material from her children and male sexual partners found in her brain? Well it is known that vaccines grown on human cells contain human DNA, even though this is truly a bad idea. Now what happens to a tiny baby when they are injected with a neurotoxic concoction or twenty at regular intervals starting right after birth. Can we hypothesize some cross sex DNA gets into the brain? I would think so. This research hasn't spelled it out yet, as far as I know, but she does explain that autism rates go up every time a new human cell line for vaccines is introduced, suggesting to me transgenderism or at least "gender dysphoria" likely goes up, too. https://soundchoice.org
I have some ideas about genetic mutations leading to cancer, too.
