Any number of ways to ‘outsource’ such activities and launder the intentional party’s true identity. I don’t think it matters much if there were actual shunts (derailing device) placed on the tracks… or if it is a statistical confluence with decades of back-longing maintenance and simple repairs… the results are the same.

I often think of the lack of difference between carelessly mistaken vs. carefully mistaken.
