Most items in the "free" world that are sold on black markets are high-value contraband that's typically associated with violence. Now that people are moar-apt to comprehend we live in a non-free world, we're going to see moar trade in things like food and medicine. I'm not sure if these markets can be controlled the way other black markets are, since there is no single source of control for the production of food and medicine - it's spread-out all over red-state America. Gaining control of that, without the cooperation of the victims, is moar difficult.
Once the victims to stop cooperating, stealing from them is hard.
