Just from a visual of the first image, it appears that the weld repair did not fail, but the base metal adjacent to the weld repair failed. To this engineer Anon, it appears that the repair was sufficient however the root cause was not addressed. Root cause could be a number of thing such as shite metal, (noted by other Anon), insufficient rail support causing flexural failure, among others. Sabotage cannot be ruled out either.
