Any access would be predicated upon legitimate hacking (yes some happens) or on compromised OS's (IOS / Android) and perhaps spreading infection from them, potentially through lower level protocols that aren't really monitored any more. Just because you're running ethernet / fiber / whatever, it doesn't mean you're necessarily seeing all comms on that meduim.
DOCSYS sniffing will give you a LOT on certain networks, and it's possible to run that over different technologies, so you can "stack" things on the same network and avoid most of the commercially available solutions' detection.
