Geisinger's MyCode Community Health Initiative Research Project

Dear XXXX,

Changes in your genes can impact your health and the health of your family. That’s why we started the MyCode Community Health Initiative.

Patients who agree to participate in MyCode give a small blood sample that allows us to look at their genes. And through this process, we’re able to identify certain health conditions earlier — before symptoms even appear — so action can be taken to keep you (and your loved ones) healthy.

Not every person who joins MyCode will hear from our team. We only share results about gene changes that can impact your healthcare right away. But your participation can help us learn more about different gene changes and the health conditions they can cause, allowing us to share more with our participants in the future.

Please reply to this message with a date and time that we can schedule a telehealth appointment with you. At this visit, you will meet with one of our MyCode team members. They will review MyCode and if you are interested can complete a consent form with their help.
