Trains derail because:

1. speed too high / no braking at turns,

2. objects on track (collision would be the word used?)

3. mechanical failure / sabotage

These seem like they were from high speed / no braking?

If most/all occurred at a turn in the track, then surely these were due to a lack of braking.

Perhaps the trains have been hacked so as to override braking procedures.
