I once saw a documentary for this. I believe its first official mission was to drill a hole into the hull of a British ship and insert explosives. Ultimately it failed its mission because the ship it chose had a reinforced hull with a metal shell or something and the diver ran out of air, having to abort and RTB. Despite this, it was still an ingenious invention and breakthrough.
