On the one hand they are incentivized by medical education and big pharma reps and continuing education. On the other hand they can't get paid, and will even lose their license, if anything they do differs from the "standard of practice" that is prevalent in their community. The peer pressure this creates is enormous. A doctor with conscience cannot go up against that immense pressure or they will be prevented from practicing medicine. The whole system is badly broken from bottom to top. Also, the idea that pharma is our friend and is looking for cures is a big lie.
