Ignorant regarding many things Military, and curious: Since when is Lieutenant General the same as "General"? Do all LT. Gens. refer to themselves as "General"?

When somebody makes the rank of General, are they OK with one rank under them also referring to themselves as "General" too, or is the military more on the "informal" side regarding Rank?
